Saskatchewan Health collects the personal information you give us so that we can provide you with a benefit, such as those provided through the Provincial Drug Plan or a service, such as registering you as a beneficiary for publicly-funded hospital and medical services. The Ministry also collects personal information from its health partners, such as regional health authorities and physicians, in order to pay for any medical or hospital service you may have received or for any other publicly-funded program or service.

Our health partners also provide us with personal information about users of their programs and services to assist the Ministry in planning, delivering, evaluating and monitoring these same programs and services. In most cases your name, address and health services number will be a part of this information.
