Visit for health information

Immunization records are maintained by the local public health office in the Saskatchewan health region where you received your immunizations.  If you have received immunization services at a First Nations community, immunization records may be at the respective clinic attended.

To locate a copy of your immunization record, public health staff will need additional information, including your date of birth, health card number, previous address, and school(s) attended. With this information, they may be able to locate the information you seek.
