Important Notice: Saskatchewan residents who have not received their health card renewal stickers should contact the Health Registration Branch. Effective midnight January 31, 2012, residents with no renewal sticker on their Saskatchewan Health Services Card may not be able to receive provincial health coverage.
 

You may apply for Saskatchewan Health Benefits for yourself, your spouse/partner and all dependents under 18 years old living in Saskatchewan. Dependants 18 years old and over must complete their own application.

You may complete your application for a Saskatchewan Health Services Card online. Electronic copies of your supporting documentation are required.

Or you can submit your application package through mail. Download an application form for a Saskatchewan Health Services card. A printed application form may be picked up from the offices of administrators of towns, villages, rural municipalities and Saskatchewan Health Registration. New residents should complete the application form and mail it together with complete supporting documentation to:

The Saskatchewan Ministry of Health issues a Saskatchewan Health Services Card to each family member who is approved for health benefits. The plastic health card is mailed just prior to the effective date of coverage.

Changes to your Saskatchewan Health Card Information

Contact Health Registration Branch if any of the following information is changed or is incorrect:

  • Contact information such as address, telephone number or email address
  • Marital status such as marriage, divorce/separation, common-law
  • Name change (marriage certificate or legal name change required)
  • Incorrect spelling of name(s)
  • Incorrect date of birth (proof is required)
  • Addition or deletions of dependants within your family unit
  • Family member attending school out-of-province
  • Extended vacation or absence from Saskatchewan of three months or more
  • Lost, stolen or damaged health services card

Change of Address or Replace Your Health Services Card Online

To report a change of address or to request a replacement health services card use ExpressAddress online service.

  • Provide your new address if moving.
  • If not moving, but your card has been lost, stolen or damaged, fill in your current address and repeat your current address for new address.
  • All family members included in the move must be entered on the form.
