Saskatchewan! Healthy people. A healthy province.

Every resident of Saskatchwan will have the opportunity to be vaccinated against the H1N1 influenza virus starting next week, without charge.

Health regions have been offering flu clinics for groups considered to be at higher risk of complications from H1N1 influenza.  See the link to Higher-risk Groups for more detailed risk criteria.

Read the information sheet at the end of the form. To save time, bring a filled-in consent form to the H1N1 flu vaccination clinic for each person getting a flu shot.
