• the person whose name appears on the change of name registration or when requested by his or her legal representative (e.g., parent, guardian, solicitor or legal agent)
• member of the immediate family if the person whose name change is recorded is deceased or incapable of making the request on his or her own behalf
• spouse of a person whose name appears on the change of name registration
• an agent, or any other person, on the written authorization of the person or the parents
• a person who requires the certificate for use in a court of law, for adoption, or for settlement of an estate
• an officer of the Crown for official purposes.
  

• Apply in person at the Vital Statistics office, by fax (if paying by credit card only) or by mail.
