Saskatchewan is spending $31 million over three years to install more safety equipment and provide training for health workers and professionals.

The Health Quality Council established a Chronic Disease Management Collaborative to facilitate quality improvements in diabetes, coronary artery disease, and access to primary care within a number of identified care facilities, practices and primary health care sites.

The Chronic Disease Management (CDM) Toolkit is used by doctors, authorized office staff and health care providers as a patient registry and decision support tool to assist in providing quality care for people with chronic conditions.

The Toolkit is used to: store and retrieve patient medical information needed to effectively plan and manage the treatment of chronic conditions; enable doctors to allow other health care providers (such as dieticians and diabetes educators) who they have identified as being directly involved in the treatment of the patient's chronic conditions to have access to the patient's health information; and to provide patient reminders for important tests, such as blood sugar or a cholesterol test so that office staff can schedule the patient for an appointment.
